Changing your personal details
We now actively record patients' sexual orientation, transgender status and ask if their gender identity is the same as the gender they were given at birth.
To change your name on your medical records, please contact us. We can also add a ‘known as’ name to your record, and make a note to add a different pronoun if you wish.
There is a process on how to change your gender too, please contact us for full information and we will go through the process with you.
Please do get in touch if you have any feedback or suggestions on how we can help improve our services further.
NHS screening programs
